BOARD AND COMMITTEE FINANCIAL DISCLOSURE REQUIREMENT FACT SHEET
Pursuant to Section 2-11.1 (i) of the Miami-Dade County Code, all autonomous personnel, quasi-judicial personnel, advisory personnel shall file, no later than 12:00 noon of July 1st of each year including the July 1st following the last year that person is in office or held such employment, one (1) of the following: a) A copy of that person’s or firm’s current federal income tax return; orb) A current certified financial statement (click to view) on a form of the type approved for use by the State or national banks in Florida listing all assets and liabilities having a value of excess or one thousand dollars ($1,000.00) and short description of each; or c) An itemized source of income statement (click to view) , under oath on a form approved by the County for said purpose. Please note that the State of Florida requires that an appointed member of certain boards of political subdivisions, including code enforcement boards, bodies with planning and or zoning powers, and pension boards fill out State FORM 1 (click to view)– Limited Financial Disclosure and will receive one in the mail prior to the filing deadline. FORM 1 (click to view) fulfills the financial disclosure requirement of BOTH the State of Florida and Miami Dade County. All other board and committee members will receive a Source of Income Statement (click to view) as well as a Financial Statement (click to view) and one must be filled out and filed with the City of Miami Beach City Clerk’s Office before the above referenced deadline.
