Disclaimer        This application is NOT your Business Tax Receipt. Do not operate the business until the Certificate of Use and the Business Tax Receipt have been issued. The application must be fully complete and submitted within thirty (days) of the start of the application process. All required documents must be attached ( i.e. state license, Articles of Incorporation, etc). If you can not upload the files, please bring them to counter to finish your application. The application will be voided if it is not completed within the 30 day period and submission of a new application will be required.
