
PASSPORT INFORMATION
Acceptance of Passport Applications:
The City of Miami Beach City Clerk's Office located at City Hall, 1700 Convention Center Drive, First Floor, accepts passport applications Monday through Friday from 9:00 AM to 4:00 PM only, excluding holidays.
Passport Renewal: Please note that Passport Renewals are not accepted.
Passport Documentation Required:
In order to assist you more expeditiously, all documents must be originals and you must provide the following documentation:
1. Two recent identical photos, 2" x 2" with a white or off white background. Passport Photos can be obtained at the City of Miami Beach City Clerk's Office at City Hall, 1700 Convention Center Drive, First Floor.
2. Proof of U.S. Citizenship
Certified U.S. birth certificate listing date recorded (long form)
Previous U.S. passport
Citizenship Certificate
Naturalization Certificate
U.S. Consular Report of Birth Abroad
If deriving citizenship from U.S. parents: proof of birth certificate (translated and notarized, if from foreign country). Also, proof of sole guardianship (translated and notarized, if from foreign country)
3. Current picture I.D.
- Driver's License
- Any other I.D. which contains a picture
PASSPORT FEES
Minors age 15 and under require both parents' or legal guardian's consent.
Additional Information:
Traveling within 16 calendar days and on emergency (death/sickness) require an appointment with the Miami Passport Agency. A new automated system allows scheduling of an appointment via telephone, (877)487-2778 or TDD (888) 874-7793 24 hours a day, seven days a week. Office is located at Claude Pepper Federal Building 51 SW First Avenue, Third Floor, Miami, Florida 33130. You must show proof of traveling and a letter from the Hospital or Funeral Home.
U.S. PASSPORT CARD
U.S. Passport card can only be used for land and sea travel between the United States and Canada, Mexico, the Caribbean and Bermuda. The U.S. Passport card cannot be used to travel by air.
