A lei exige que voc\u00ea comunique o acidente ou o seu conhecimento de uma les\u00e3o\/doen\u00e7a relacionada ao trabalho no prazo de 30 dias a partir do seu conhecimento do acidente ou les\u00e3o\/doen\u00e7a.<br \/>\n\u2022 Para seu atendimento inicial, utilize o centro de tratamento autorizado, Fast Care, a menos que o ferimento\/doen\u00e7a seja grave. Nesse caso, dirija-se ao pronto-socorro mais pr\u00f3ximo.<br \/>\n\u2022 Seu supervisor relatar\u00e1 a les\u00e3o\/doen\u00e7a \u00e0 CorVel Corporation por meio do Sistema de gerenciamento de reivindica\u00e7\u00f5es da CareMC.<br \/>\n\u2022 Seu supervisor continuar\u00e1 recebendo atualiza\u00e7\u00f5es do Examinador de Reivindica\u00e7\u00f5es da CorVel sobre o status de retorno ao trabalho, restri\u00e7\u00f5es, agendamentos e libera\u00e7\u00e3o quando um funcion\u00e1rio estiver apto a retornar ao trabalho. Quaisquer d\u00favidas ou preocupa\u00e7\u00f5es relacionadas \u00e0 reivindica\u00e7\u00e3o devem ser encaminhadas primeiro ao Examinador de Reivindica\u00e7\u00f5es da CorVel.<br \/>\n\u2022 Dentro de 3 a 5 dias \u00fateis ap\u00f3s a comunica\u00e7\u00e3o do acidente ou les\u00e3o\/doen\u00e7a, voc\u00ea dever\u00e1 receber um folheto informativo explicando seus direitos e obriga\u00e7\u00f5es, e uma Carta de Notifica\u00e7\u00e3o explicando os servi\u00e7os prestados pelo Escrit\u00f3rio de Assist\u00eancia ao Funcion\u00e1rio da Divis\u00e3o de Compensa\u00e7\u00e3o Trabalhista. Esses formul\u00e1rios podem fazer parte de um pacote que pode incluir alguns ou todos os seguintes itens:<br \/>\nUma c\u00f3pia do seu relat\u00f3rio de acidente ou \u201cPrimeiro Relat\u00f3rio de Les\u00e3o ou Doen\u00e7a\u201d, que voc\u00ea deve ler para ter certeza de que est\u00e1 correto;<br \/>\nUma declara\u00e7\u00e3o de fraude, que voc\u00ea deve ler, assinar e devolver o mais r\u00e1pido poss\u00edvel, ou os benef\u00edcios poder\u00e3o ser temporariamente retidos at\u00e9 que voc\u00ea o fa\u00e7a;<br \/>\nUma libera\u00e7\u00e3o de registros m\u00e9dicos para voc\u00ea assinar e devolver; e<br \/>\nFormul\u00e1rios de reembolso de quilometragem m\u00e9dica que voc\u00ea deve preencher ap\u00f3s procurar tratamento m\u00e9dico e enviar ao seu Analista de Sinistros da CorVel para reembolso. Caso n\u00e3o receba uma liga\u00e7\u00e3o ou o pacote de informa\u00e7\u00f5es da CorVel Corporation, ligue para a linha direta do WC para obter assist\u00eancia pelo telefone 1.800.342.1741.<br \/>\n\u2022 D\u00ea ao m\u00e9dico uma descri\u00e7\u00e3o completa do acidente ou de como voc\u00ea se feriu.<br \/>\n\u2022 Se voc\u00ea for liberado para trabalhar, mas n\u00e3o puder retornar ao seu antigo emprego, voc\u00ea deve obter instru\u00e7\u00f5es do m\u00e9dico sobre o trabalho que voc\u00ea pode ou n\u00e3o fazer.<br \/>\n\u2022 Compare\u00e7a a todas as consultas m\u00e9dicas, ou os benef\u00edcios poder\u00e3o ser suspensos.<br \/>\n\u2022 Fale com seu supervisor assim que sair do consult\u00f3rio m\u00e9dico. Explique a ele o trabalho que o m\u00e9dico disse que voc\u00ea pode e n\u00e3o pode fazer.<br \/>\n\u2022 Pergunte ao seu empregador se ele tem algum trabalho para o qual voc\u00ea possa retornar que n\u00e3o exija que voc\u00ea fa\u00e7a coisas que o m\u00e9dico disse que voc\u00ea ainda n\u00e3o pode fazer.<br \/>\n\u2022 Se sim, pergunte quando voc\u00ea deve se apresentar para trabalhar.<br \/>\n\u2022 Caso contr\u00e1rio, certifique-se de que seu empregador tenha uma maneira de entrar em contato com voc\u00ea caso haja uma vaga de trabalho adequada dispon\u00edvel.<br \/>\n\u2022 Se voc\u00ea for internado em um hospital, ligue ou pe\u00e7a para algu\u00e9m ligar para seu Supervisor para explicar o que aconteceu e onde voc\u00ea est\u00e1.<br \/>\n\u2022 Entre em contato com o examinador de reivindica\u00e7\u00f5es da CorVel e informe o que o m\u00e9dico disse sobre seus ferimentos e situa\u00e7\u00e3o de trabalho.<br \/>\n\u2022 Voc\u00ea deve continuar em contato com seu supervisor e seu examinador de reivindica\u00e7\u00f5es da CorVel durante todo o seu tratamento e recupera\u00e7\u00e3o.\n<b>DEVERES DO EMPREGADOR<\/b><br \/>\n\u2022 Entre em contato com a CorVel Corporation imediatamente, por meio do Sistema de Gerenciamento de Reivindica\u00e7\u00f5es da CareMC, para relatar uma les\u00e3o\/doen\u00e7a relacionada ao trabalho.<br \/>\n\u2022 Mantenha contato com seu funcion\u00e1rio e com o examinador de sinistros da CorVel at\u00e9 que o trabalhador lesionado retorne ao trabalho.
