- Approved Certificate of Use
- Recorded Warranty Deed or Bill of Sale
- Articles of Incorporation
- Federal Id Number
- Florida Annual Resale Certificate for Sales Tax (Annual Resale Certificate)
- State License
- Letter from the Association that short term rental is allowed for your specific unit(s), dated within the last 60 days. Please note: if the Association does not have an active BTR, they are required to have their BTR approved prior to the release any Short-Term Rental BTRs for individual unit(s) that has applied.
- Written notice that Property Owner has received, reviewed, and recognized that the use of the residential property as a short-term residential rental could result in the loss of the homestead exemption
- Provide the name and contact information for all hosting platforms, advertisement platforms and short-term residential rental advertising platforms on which the residential property is, or will be, listed for rent
- Resort tax registration
- Photo ID and signatures of all operators (authorized members) on application
- Notarized Affidavit

- The original affidavit must be submitted via mail or in person
