Hotel Requirements 

Required Documents:

• ​​Approved Certificate of Use
• ​Executed Lease Agreement or Recorded Warranty Deed or Bill of Sale
• Articles of Incorporation
• ​Federal Id Number
• ​Fictitious Name Registration
• Miami-Dade Department of Regulatory and Economic Resources - Municipal Certificate of Use (send the application form to the Building Department by submitting a records request)
• State License
• Safety Button Affidavit
• ​Number of Units
• ​Resort Tax Registration  Copy of Driver's license of owner(s) on record. Note: if the owner is a corporation the identification is required for all officers listed in the corporation.

The following codes may apply:

• Occupational Code: 95009500 – Hotel 
• Occupational Code: 95018550 – Swimming Pool 
• Occupational Code: 95016400 – Restaurant/Bar 

Notes:

• If a Restaurant/Bar/Cafe exists for the same entity, it can be included in the same application.
• If a Restaurant/Bar/Cafe exists for a different entity, a separate application is required.
