Miami-Dade Department of Regulatory and Economic Resources - Municipal Certificate of Use (send the application form to the Building Department by submitting a records request)

Notes:

If the occupant load exceeds 200 it’s necessary to charge a Nightclub Load Fee. The number of persons (not seats) should be indicated on the Business Tax Receipt.
