Passport Renewal:
Please note that Passport Renewals are NOT accepted. NOTE: Passports issued to minors are not considered renewals. In this case, you must submit a new DS-11 application.

Passport Documentation Required:
Completed U.S. Passport form in black ink only.  Please do not sign the passport application until asked to do so by the Acceptance Agent.  Passport forms are available in our Office or online through the U.S. Department of State's Website.

  • All passport applicants must appear in person, including minors.
  • Applicants must submit evidence of U.S. Citizenship when applying.
    • If born in the United States proof of citizenship may be in the form of a
      Certified Copy of a U.S. Birth Certificate or a previously issued U.S.
      Passport Book or Card. (For information on obtaining a Certified Copy of a Birth
      Certificate please click here)
    • Applicants born abroad may submit a Naturalization Certificate, Consular
      Report of Birth Abroad, or a previously issued U.S. Passport Book or Card.
    • Puerto Rican born applicants may use their Birth Certificates as evidence of
      Citizenship as long as they were issued on or after July 1, 2010.

     

  • All Applicants must present acceptable identification.  The
    following documents are acceptable as form of identification:

    • Valid Driver's License
    • Official U.S. Military Identification Card
    • Government Employee Identification Card
    • Valid Foreign Passport
    • FAA Pilot's License

     

  • One Passport Photograph. All passport photographs must meet the
    following criteria:

    • In Color
    • 2 x 2 inches in size
    • Taken within the past 6 months, showing current appearance
    • Full face, front view
    • On a plain white or off-white background
    • Photograph must be taken in normal street attire

     

  • Minors
    All children regardless of age, including newborns and infants
    must be present when applying for a U.S. Passport.  To execute the application
    of a minor both parents must appear with the minor and submit:
  • Form DS-11 completed in black in only, please do not sign the
    Form
  • All Applicants under the age of 18, must present an Original Birth
    Certificate.  (If born outside of the U.S. a translated copy is also required)
  • Evidence of the minor's U.S. Citizenship (Certified Copy of a Birth
    Certificate, Report of Birth Abroad or previously issued us Passport Book or
    Card)
  • Evidence of parental relationship (Original or Certified Copy of a Birth
    Certificate, Certified Adoption Decree, Certified Court Order.  Note: Documents
    must be translated if in a language other than English)
  • Personal photo identification of the parents
  • One passport photo of the minor
  • If only one parent is present, the second parent's notarized consent (Form DS-3053) must be submitted with a photocopy of his/her ID
    (front/back)
  • Minors age 16 or 17 must show identification, have one parent present and
    show evidence of the parental relationship.

PASSPORT FEES

ROUTINE SERVICE (6-8 weeks)
For applicants 16 and older:

  • $130 passport book fee payable to U.S. Department of State via check or money order
  • $35 in addition for the execution fee in cash, check or credit card, payable to the City of Miami Beach

For applicants under age 16:

  • $100 passport book fee payable to U.S. Department of State via check or money order
  • $35 in addition for the execution in cash, credit card or check, payable to the City of Miami Beach

Expedited Service (2-3 weeks)

U.S. PASSPORT BOOK

  • Adults Expedited Service = $130 + $60= $190 payable to U.S. Department of State via check or money order
  • Minors Expedited Service = $100 + $60= $160 payable to U.S. Department of State via check or money order
  • For overnight optional delivery to U.S. Department of State=$26.95 in cash, credit card or check payable to the City of Miami Beach
  • For 1-2 day optional delivery of your passport to your residence=$19.53 to U.S. Department of State via check or money order
  • U.S. Passport cards can only be used for land and sea travel between the United States and Canada, Mexico, the Caribbean and Bermuda. The U.S. Passport card cannot be used to travel by air.

**Passport Cards are an additional $30.00 for Adults and $15.00 for Children**

Passport Photo fee:
$12 passport photo fee payable in cash, credit card or check, payable to the City of Miami Beach

Additional Information:

There are two types of expedited appointments: Life-or-Death Emergency Service and Urgent Travel Service. You cannot walk-in. There is no charge to make an appointment at a Passport Agency or Center.

You cannot transfer your appointment to another customer. Learn more about this change to make it easier for you to make an appointment.

The Miami Passport Office is located at 1501 Biscayne Boulevard, Suite 400, Miami, Florida, 33132. You must show proof of traveling and a letter from the Hospital or Funeral Home.
